Why Does Root Canal Treatment Fail?

Root canal treatment (endodontic treatment) can sometimes fail and there can be several reasons for this. Common reasons for a failed root canal treatment can be:

Inability to Completely Clean the Infection: During root canal treatment, the infected pulp tissue (nerve and vessels) is removed and the canals are cleaned. However, failure to remove all infected tissue or inadequately clean the canals during this procedure can cause the infection to persist.
Crooked or Complex Canals: Some teeth have complex canals and it can be difficult to completely clean and fill these canals. This can lead to infected tissue residues remaining.
Inadequate Filling: Failure to adequately fill or seal the canals during root canal treatment can cause bacteria to reenter the canals and the infection to reoccur.
Incorrect Treatment Planning: The dentist may have made the wrong decision about which tooth to treat. Treating the wrong tooth can lead to the loss of the tooth and the infection to persist.
Repeated Trauma: A tooth that has undergone root canal treatment can be damaged by repeated trauma or forceful blows, rendering the treatment unsuccessful.

New Infection: A new infection in the tooth can occur after a previously successful root canal treatment.

Factors Depending on the Condition of the Tooth: In some cases, root canal treatment can be difficult due to the anatomy or health of the tooth, and it can be more difficult to achieve a successful result.

Poor Workmanship: A dentist’s mistake during root canal treatment or poor quality workmanship can lead to unsuccessful results.

In the event of an unsuccessful root canal treatment, a repeat root canal treatment (re-root canal treatment) can be performed, but this is not always possible or recommended. Other alternatives include root end resection (apicotomy) or tooth extraction. The best approach is careful diagnosis and treatment planning for successful root canal treatment, and therefore should be performed by a specialist dentist or endodontist.
